Download instructions: Click the link below the form you want to download, the link will take you off the NYMM Series site to a FileFactory site.  Scroll down to the bottom of the page where you will find a spot "DOWNLOAD NOW" with 2 green down arrows.  Click that button.  It will then take you to a second page, scroll to the bottom, you will see "YOUR DOWNLOAD WILL BEGIN IN 30 SECONDS" and it will start to count down.  At 0, it will now read "Download with Filefactory Basic", click that and you will recieve your download.   Sorry for the difficulty, I am in the process of trying to find an easier way to post the forms but as of now, this is the only way I have available.
